    Currently seeking a Case Manager, RN, to work in our Case Management Department who will provide service excellence and support the hospital's mission, vision and values through appropriate conduct and demeanor at all times.  The individual in this position has overall responsibility for overseeing the clinical plan of care to conform to evidence-based practice and regulatory requirements.  This position integrates care coordination, utilization management, and discharge planning.


    *** FULL TIME, 40 hours per week***


    The individual's responsibilities will include, but not be limited to the following activities:

    • Medical necessary screening/millimam utilized
    • Care coordination
    • Discharge planning
    • Facilitating multi-disciplinary patient care conferences
    • Managing concurrent disputes
    • Making the appropriate referrals to other departments (nutrition, PT/OT/ST)
    • Communicating with patients and their families about the plan of care
    • Collaborating with physicians, office staff and ancillary departments (lab, pharmacy)
    • Utilization review
    • Participating in Complex Case Review
    • Arranging for post-discharge patient education
    • Clear, complete and concise documentation in EHR (EPIC)
    • Maintaining accuracy of patient demographic and insurance information
    • Identifying and documenting potentially avoidable days
    • Identifying and reporting over and under utilization
    • Other duties as assigned


    Requires knowledge of Federal, State and required regulatory agency standards related to health care organizations.  Cognitive and technical knowledge and ability to progressively manage case load is required; The ability to retrieve, communicate and present data and information both verbally and in writing is required as in the ability to express or exchange ideas by means of the spoken and written word.  Must be flexible with work scheduling.  Involves discretion and independent action within prescribed limits.

    • Associate's Degree Required, Bachelors Degree preferred
    • Current TX RN License Required
    • 1-3 years Case Management experience OR 3-5 years recent bedside clinical nursing is required
    • Excellent organizational skills, and verbal and written communication skills
    • Ability to lead and coordinate activities of a diverse group of people
    • Demonstrated critical thinking and problem solving skills
    • Experience with electronic and case management documentation systems, helpful
    • BLS from American Heart Association
