Currently seeking an LVN, Discharge Advocate, to work in our Case Management Department who will provide service excellence and support the hospital's mission, vision and values through appropriate conduct and demeanor at all times.

 

Responsibilities

Primary responsible for maintaining effective customer service for all internal and external THRPHR customers by utilizing excellent, in-depth knowledge of company services and programs as well as team members within the Customer Service Department. Supervises and trains Customer Service Representatives. Reviews, researches and evaluates complaints and inquiries. Prioritizes concerns in multiple or complete issues. Oversees referrals, investigation, and resolution of complaints. Responds to government or regulatory agencies. Identifies problems and works with those involved to implement changes and solutions. Follows THRPHR Grievance Management Guidelines. Acts as advocate for patients and makes referrals to appropriate hospital or community resources. Acts as liasion between patients, staff and physicians. Serves as resource person for hospital staff, medical staff and physician's office staff. Documents of complaints received. Provide data for centralized database of patient complaints and assists with reporting of patient satisfaction/patient complaints as requested by entity management.

Qualifications

Requires knowledge of Federal, State and required regulatory agency standards related to health care organizations.  Cognitive and technical knowledge and ability to progressively manage case load is required; The ability to retrieve, communicate and present data and information both verbally and in writing is required as in the ability to express or exchange ideas by means of the spoken and written word.  Must be flexible with work scheduling.  Involves discretion and independent action within prescribed limits.

  • Current TX LVN License Required
  • Discharge Advocate experience is preferred
  • 2 years healthcare experience is preferred
